Cleveland, Ohio clinic performs US’s first face transplant

A team of eight transplant surgeons in Cleveland Clinic in Ohio, USA, led by reconstructive surgeon Dr. Maria Siemionow, age 58, have successfully performed the first almost total face transplant in the US, and the fourth globally, on a woman so horribly disfigured due to trauma, that cost her an eye. Two weeks ago Dr. Siemionow, in a 23-hour marathon surgery, replaced 80 percent of her face, by transplanting or grafting bone, nerve, blood vessels, muscles and skin harvested from a female donor’s cadaver.

The Clinic surgeons, in Wednesday’s news conference, described the details of the transplant but upon request, the team did not publish her name, age and cause of injury nor the donor’s identity. The patient’s family desired the reason for her transplant to remain confidential. The Los Angeles Times reported that the patient “had no upper jaw, nose, cheeks or lower eyelids and was unable to eat, talk, smile, smell or breathe on her own.” The clinic’s dermatology and plastic surgery chair, Francis Papay, described the nine hours phase of the procedure: “We transferred the skin, all the facial muscles in the upper face and mid-face, the upper lip, all of the nose, most of the sinuses around the nose, the upper jaw including the teeth, the facial nerve.” Thereafter, another team spent three hours sewing the woman’s blood vessels to that of the donor’s face to restore blood circulation, making the graft a success.

The New York Times reported that “three partial face transplants have been performed since 2005, two in France and one in China, all using facial tissue from a dead donor with permission from their families.” “Only the forehead, upper eyelids, lower lip, lower teeth and jaw are hers, the rest of her face comes from a cadaver; she could not eat on her own or breathe without a hole in her windpipe. About 77 square inches of tissue were transplanted from the donor,” it further described the details of the medical marvel. The patient, however, must take lifetime immunosuppressive drugs, also called antirejection drugs, which do not guarantee success. The transplant team said that in case of failure, it would replace the part with a skin graft taken from her own body.

Dr. Bohdan Pomahac, a Brigham and Women’s Hospital surgeon praised the recent medical development. “There are patients who can benefit tremendously from this. It’s great that it happened,” he said.

Leading bioethicist Arthur Caplan of the University of Pennsylvania withheld judgment on the Cleveland transplant amid grave concerns on the post-operation results. “The biggest ethical problem is dealing with failure — if your face rejects. It would be a living hell. If your face is falling off and you can’t eat and you can’t breathe and you’re suffering in a terrible manner that can’t be reversed, you need to put on the table assistance in dying. There are patients who can benefit tremendously from this. It’s great that it happened,” he said.

Dr Alex Clarke, of the Royal Free Hospital had praised the Clinic for its contribution to medicine. “It is a real step forward for people who have severe disfigurement and this operation has been done by a team who have really prepared and worked towards this for a number of years. These transplants have proven that the technical difficulties can be overcome and psychologically the patients are doing well. They have all have reacted positively and have begun to do things they were not able to before. All the things people thought were barriers to this kind of operations have been overcome,” she said.

The first partial face transplant surgery on a living human was performed on Isabelle Dinoire on November 27 2005, when she was 38, by Professor Bernard Devauchelle, assisted by Professor Jean-Michel Dubernard in Amiens, France. Her Labrador dog mauled her in May 2005. A triangle of face tissue including the nose and mouth was taken from a brain-dead female donor and grafted onto the patient. Scientists elsewhere have performed scalp and ear transplants. However, the claim is the first for a mouth and nose transplant. Experts say the mouth and nose are the most difficult parts of the face to transplant.

In 2004, the same Cleveland Clinic, became the first institution to approve this surgery and test it on cadavers. In October 2006, surgeon Peter Butler at London‘s Royal Free Hospital in the UK was given permission by the NHS ethics board to carry out a full face transplant. His team will select four adult patients (children cannot be selected due to concerns over consent), with operations being carried out at six month intervals. In March 2008, the treatment of 30-year-old neurofibromatosis victim Pascal Coler of France ended after having received what his doctors call the worlds first successful full face transplant.

Ethical concerns, psychological impact, problems relating to immunosuppression and consequences of technical failure have prevented teams from performing face transplant operations in the past, even though it has been technically possible to carry out such procedures for years.

Mr Iain Hutchison, of Barts and the London Hospital, warned of several problems with face transplants, such as blood vessels in the donated tissue clotting and immunosuppressants failing or increasing the patient’s risk of cancer. He also pointed out ethical issues with the fact that the procedure requires a “beating heart donor”. The transplant is carried out while the donor is brain dead, but still alive by use of a ventilator.

According to Stephen Wigmore, chair of British Transplantation Society’s ethics committee, it is unknown to what extent facial expressions will function in the long term. He said that it is not certain whether a patient could be left worse off in the case of a face transplant failing.

Mr Michael Earley, a member of the Royal College of Surgeon‘s facial transplantation working party, commented that if successful, the transplant would be “a major breakthrough in facial reconstruction” and “a major step forward for the facially disfigured.”

In Wednesday’s conference, Siemionow said “we know that there are so many patients there in their homes where they are hiding from society because they are afraid to walk to the grocery stores, they are afraid to go the the street.” “Our patient was called names and was humiliated. We very much hope that for this very special group of patients there is a hope that someday they will be able to go comfortably from their houses and enjoy the things we take for granted,” she added.

In response to the medical breakthrough, a British medical group led by Royal Free Hospital’s lead surgeon Dr Peter Butler, said they will finish the world’s first full face transplant within a year. “We hope to make an announcement about a full-face operation in the next 12 months. This latest operation shows how facial transplantation can help a particular group of the most severely facially injured people. These are people who would otherwise live a terrible twilight life, shut away from public gaze,” he said.

In the Opening Day of 2008 AutoTronics Taipei (April 9), the Industrial Technology Research Institute of Taiwan (ITRI) invited Jamie C. Hsu (Consultant of Ministry of Economic Affairs of the Republic of the China and Former Management Executive Director of General Motors Global Technology) for a speech in the “Pavilion of Taiwan Automotive Research Consortium (TARC)” to forecast the future of automobile industry in Taiwan, which echoed the “Taiwan Automotive International Forum & Exhibition”, held at TWTC Nangang in conjunction with 2008 Taipei AMPA.

Before the main show, Department of Industrial Technology of Ministry of Economic Affairs supervised the establishment of TARC by ITRI, Chung-Shan Institute of Science and Technology, Metal Industries Research & Development Center, and the other academical and industrial units to improve the level of research and development including applications on security, artificial intelligence, and energy-saving.

There were several crisis for the automobile industry in Taiwan because of the decrease of market scale, low self-independence, technology transition, and the rise up of oil prices. But after in conjunction with light-weighting, electronical, and energy-saving related industries, there were other chances and challenges for the automobile industry in Taiwan. Currently, the Taiwan Automotive Research Consortium should do a proper role on R&D, policy driving, and quality improving, even though urban and rural differences, and key issue of carbon dioxide wasting, if there is a new innovation, Taiwan will be a good example in the automobile industry in the world.

Online buyers conned by fake Olympics ticket web sites

Two web sites purporting to sell tickets to the 2008 Summer Olympic Games due to open in Beijing, China on Friday have been the subject of lawsuits from the International Olympic Committee in United States courts in recent weeks. The web sites, beijingticketing.com and beijing-2008tickets.com, were designed to resemble official sites and often appeared above the official sites in Google searches, and scammed some victims out of up to US$50,000 each for tickets to events such as the Opening Ceremony and swimming, which were listed despite the official Beijing ticket web site stating that tickets to all events had sold out as of July 27. The sites are believed to have taken in millions of dollars in total.

Ken Gamble, a private investigator from Sydney, Australia, believes the sites are operated by Terance Shepherd, whom Gamble has been tracking for several years in relation to other fraudulent web sites selling tickets to events such as the FIFA World Cup. According to Gamble, Shepherd’s modus operandi involves setting up the fake web sites, overselling the tickets, and then failing to produce the tickets. “The story’s always the same,” Gamble said, “it’s an ‘unfortunate mistake’ or someone has ‘let them down’. They promise a refund, which never happens, and the credit companies end up paying all the refunds.” Shepherd owns a home in the London suburb of Blackheath, although authorities believe he is currently hiding in Barbados.

On Monday, beijingticketing.com customers received an email from someone named “Alan Scott”, claiming that the site’s ticket supplier had filed for bankruptcy. The email recommended that customers should contact their credit card companies to seek refunds “immediately”, and said that the company would set up a call centre to provide assistance to its customers. However, the site has been shut down and a phone number previously listed on it has been disconnected. The company’s address, in Phoenix, Arizona, was found to be an empty office space. Visa International has stated that victims who used their Visa card to pay should be able to get their money back, but it is not yet clear whether the same will apply to all customers.

You can surely bet your short ribs I know a thing or two about Barbeque. If it’s true, it ain’t bragging.

You could wake me from a dead slumber and I’ll mix a batch of barbeque sauce that’ll reform your life. I know the best sugar and white vinegar sauce, the very best tomato base sauce and the best absolutely-to-die-for traditional Carolina type mustard base sauce. Don’t bother taking me to the hospital if I’m ever in a car wreck and need a transfusion. These guys can not help me. Get me to the closest authentic, down home barbecue joint and plug me right into a bottle of barbecue sauce.

I’ll admit, when many people think about Florence, they don’t consider highbrow cuisine. They most likely think about tobacco or cotton or the dang highway as well as just getting on down the road. I’m just not going to reveal to you what Chicken Bog is. You need to find out on your own. I’ll simply just say you do not want to miss it and you can’t find it everywhere else.

Now, permit me to tell you about the real barbecue restaurants in Florence South Carolina. A few of them are almost famous, but a few of the very best one’s are supersecret. I am really just going to share with you about the two finest ones.

Philadelphia breaks 80 year old building lease; moves to evict Scouts

In an unannounced vote yesterday, the Philadelphia City Council in the United States voted 16-1 to endorse the eviction of, and end their lease held in perpetuity with, the local council of the Boy Scouts in Philadelphia. The Scouts must pay market rent or leave the building. The Cradle of Liberty Council has more than 60,000 members in Philadelphia and Delaware and Montgomery Counties.

EU bans all Indonesian airlines as well as several from Russia, Ukraine and Angola

The European Union banned all of Indonesia’s air carriers yesterday, none of which presently operate services to Europe, as well as several from Russia, Ukraine and Angola. They are the latest additions to the already extensive List of air carriers banned in the EU. The ban is scheduled to come into effect on July 6. Just hours after the ban a Boeing 737 operated by one of the blacklisted airlines, TAAG Angola Airlines, crashed into a house during landing, causing at least six fatalities in Northern Angola.

Indonesia currently has 51 airlines, having grounded several and revoked the licences of others on June 25. The EU said that substandard maintenance and operation and a slow reaction by Indonesia to solve the problem were the main causes of the ban. EU holidaymakers who have booked flights with banned airlines via travel agents will be refunded for the services.

EU transport commissioner Jacques Barrot said of the ban “Once more, the EU blacklist will prove to be an essential tool not only to prevent unsafe airlines from flying to Europe and to inform passengers travelling worldwide, but also to make sure that airlines and civil aviation authorities take appropriate actions to improve safety.”

Operations and safety editor at Flight International David Learmount commented that Indonesia, whose airline industry was deregulated the early 1990s, is one of a handful of cases where deregulation has lowered safety standards instead of improving them, saying of the move by the EU “Standards in aviation safety have been going up dramatically on a worldwide basis, but there are still places where they are [of the standards of] the 70s and 80s. In Indonesia the safety watchdog was told earlier this year to pull its socks up, but the EU is clearly convinced that it has not done so.”

One unnamed EU official was reported by The Guardian to have described Indonesia’s civil aviation authority as “not very reliable”, referring to a lack of reaction to warnings of an imminent ban and requests that Indonesia reassured officials that the problem was being dealt with.

Indonesia has responded to the ban by saying that, according to information unseen by the EU, Indonesian safety standards are rising. Director-general of civil aviation at the Indonesian transport ministry Budhi Mulyawan Suyitno told Reuters new agency that, “Our data can show them that we have improved on every line. The US had already downgraded Indonesia’s safety rating earlier this year.

Also affected by the bans are Ukraine’s Volare Aviation, while Russia has imposed bans on four of its airlines after consulting the EU and restricted six others, Bulgaria has revoked the licences of six cargo airlines and Moldova has banned eight airlines.

Meanwhile, Pakistan International Airlines, subject of a controversial EU ban earlier this year, had restrictions on some of its aircraft lifted. The airline’s fleet of Boeing 777s and some of their Boeing 747s and Airbus 310s will now be allowed back into European airspace.

The announcements come after three accidents involving Indonesian airliners – the New Year’s Day crash of Adam Air Flight 574, which killed 102 people, the subsequent accident involving Adam Air Flight 172, which cracked in half on a hard landing but held together, preventing serious injury, and the March crash of state-run Garuda Indonesia Flight 200, which claimed 23 lives. All the accidents involved ageing Boeing 737 aircraft.

California governor Arnold Schwarzenegger proposed budget cuts and layoffs for California yesterday.

Californian education budgets would be cut by $3 billion USD for the next five years if California voters gave a yes votes to the budget-related measures for May 19’s special election.

Schwarzenegger is also ready to sell state properties including the San Quentin State Prison and the Los Angeles Coliseum to raise money for the state.

Nintendo DS sales hit one million units last April. However, even with Nintendo’s moderate success with their DS gaming system, all video game sales have declined 17%.

GameSpot writer Tor Thorsen claims that Nintendo DSi, the current release of the Nintendo DS, is an acronym for “Dominating Sales in America.”

Eight-time gold medalist Michael Phelps, in a recent conference Thursday, told reporters he will enter a swimming competition today.

This is Phelps’ first swim meet since he was photographed with a marijuana pipe in his mouth.

Phelps will swim the 200-meter freestyle and 100-meter butterfly races.

After the marijuana pipe photo was released, Phelps was unsure of whether he would swim again.

If the amount of bail set by the court is more than can be paid by the accused, he or she will approach an agency that provides bail bonds in Minnesota. The bail bond agency will demand that the accused put up a percentage of the amount personally, usually 15 percent. The remainder is put up by the bail bonding agency. This money is designed to ensure that the accused appears in court on the day specified by the judge, if the individual does not show up; the bail bondsman will hire a bounty hunter who will attempt to locate and bring him into court. If this individual is successful a monetary reward will be paid by the agency that provided the bail bonds in Minnesota in the first place.

When a person finds himself arrested and accused of committing a crime, there is a time period between the date of the arrest and the date set for a court appearance. Rarely would the accused want to stay incarcerated during this period so bail for temporary freedom is set. One of the underlying premises of the US justice system is that the accused is considered innocent until proven otherwise so a judge will let the accused out of jail on bail. This temporary release is contingent on the accused being able to put up the amount of money as collateral for return to court.

Death toll of Bahrain tourist boat capsizing rises to 57

Saturday, April 1, 2006At least 57 people are now known to have died when a boat designed similar to a traditional dhow capsized one mile of the coast of Bahrain in the Persian Gulf. Seventeen were Indian and a further 13 of the dead are British; people of another six nationalities are also known to have died. The passengers were mostly foreigners from the Nass-Murray and Roberts construction company, and were enjoying an evening dinner cruise in celebration of their completion of part of the Bahrain World Trade Centre towers. Most who died were dining on the lower deck, and were trapped below.

Over 130 people were onboard the boat at the time, although some say the boat was only licensed to carry 120. Others say the limit was 150. Eyewitnesses say the boat was overloaded. The boat capsized in calm seas soon after it left, trapping many below. Helicopters from the US Navy searched for 13 missing persons until Friday morning, when the search was called off. The dhow had been modified to have an extra, higher deck making the vessel less stable. The exact cause is unknown – suggestions from eyewitnesses include the boat suddenly turning left, being hit by a wave, and many of the passengers moving to one side of the boat together. The Bahrain Interior Ministry is to investigate the accident and establish if the boat was seaworthy.

Canadian top court strikes down private medicare ban in Quebec

Canada’s top court has struck down Quebec’s bans on private health care insurance, citing an increased risk to the life and health of Canadians. [1]

The Supreme Court of Canada ruling looked into a patient’s right to pay for faster service in a system that currently treats patients on the basis of equal access to medical care, regardless of income. [2]

Quebec patient George Zeliotis, a chemical salesman who waited in pain for more than a year in 1997 to have his hip replaced, said he should have had the right to pay for surgery.

Under public health care, it’s forbidden to pay for services covered under the system.

Despite free medical treatment, there are often long waiting lists for operations and services with current public health care.[3]

Together with physician, Dr. Jacques Chaoulli, Mr. Zeliotis launched a challenge to the Supreme Court of Canada, after losing their fight in Quebec’s lower courts, arguing that having to wait for surgery violates a patient’s constitutional right to life, liberty, and security of the person. [4]

Mr. Zeliotis and Dr. Chaoulli argued that being able to pay for private medical services wouldn’t be detrimental to the public health care system.

The Quebec Superior Court and the Quebec Court of Appeal had dismissed the case, ruling that the provincial law’s intent was not to discriminate among patients and to provide health care based on need rather than a patient’s ability to pay.

The Canadian Medical Association said the Superior Court of Canada ruling could “fundamentally change the health-care system in Canada as we now know it” but declined to comment any further until it had time to study the decision. [5]
